Case 1 

Patient History:

This patient exhibits a classic Keratoconus shape and cornea thickness. Development of this cornea shape has occurred over the past twenty five years. Correction with eyeglasses has not been an option due to the high irregularity of the cornea surface in the optic zone area. Cornea thinning is marked in the lower right hand diagram.

Tangential Keratometry Map

Correction Options:

In the early stages, this patient wore a tricurve gas permeable Keratoconus lens. As the cone progressed in the central region an aspheric nipple design gas permeable Keratoconus was fitted with limited comfort. The patient proceeded to wear the nipple design with a soft piggy back lens.

The current fitting was upgraded to a hybrid soft/gas permeable lens designed for Keratoconus with comfort lasting ten hours a day. For additional comfort, a pair of toric soft lenses were fit with a .16 center thickness to increase lens modulus and enhance vision correction. Visual acuity with the hybrid lens is 20/20 and in the soft toric 20/30.

Case 2 

Patient History:

This 21 year old patient presents with a unilateral Keratoconus. Onset began in his late teens. Progression has been rapid. His current pachymetry is 336 microns. Under slit lamp examination, vertical striated cells are easily seen along with the apical opacity of 1 1/2 mm. * Note the scale on the Keratometry. His reading of 58.5 diopters requires the scale to be increased.

3- Dimensional Map 

Correction Options:

The 3-D map to the left shows the cone well. With the rapid change of cornea specifications, this patient requires constant updates. He is currently wearing a "nipple" design Keratoconus Gas Permeable. He sometimes has discomfort with the apex of his cone suffering abrasion and requires a piggy back soft lens to aid in healing. A new hybrid soft/gas permeable for Keratoconus is being investigated for this patient, but results are similar to the gas permeable and piggy back soft. Vision acuity is 20/30.
